Job Description

An exciting opportunity to work as an integral part of the Nephrology multidisciplinary team, providing care to patients with chronic kidney disease (CKD) through innovation and excellence.

Responsibilities

  • Provide effective and timely care to patients with CKD through education and clinical support, including an NP led supportive care clinic.
  • Demonstrate advanced, holistic and comprehensive nursing practice to promote optimal health and best outcomes for patients, care givers and family.
  • Work as part of a diverse multipisciplinary team, including Nephrologists, acute renal team, palliative care service, primary health and community-based services.

Qualifications

  • Registration as a Registered Nurse with AHPRA
  • Post graduate Masters of Nurse Practitioner qualification, or a Masters assessed as equivalent in qualification
  • A minimum of 5-years experience in Nephrology or renal nursing
  • Demonstrated ability to practice collaboratively as part of a multidisciplinary health care team, providing high quality patient care
  • Advanced assessment, problem solving, critical thinking and organisational skills
  • Excellent communication and interpersonal skills
