Job Description

An exciting opportunity to work as an integral part of the Nephrology multidisciplinary team, providing care to patients with chronic kidney disease (CKD) through innovation and excellence.

Responsibilities

Provide effective and timely care to patients with CKD through education and clinical support, including an NP led supportive care clinic.
Demonstrate advanced, holistic and comprehensive nursing practice to promote optimal health and best outcomes for patients, care givers and family.
Work as part of a diverse multipisciplinary team, including Nephrologists, acute renal team, palliative care service, primary health and community-based services.

Qualifications

Registration as a Registered Nurse with AHPRA
Post graduate Masters of Nurse Practitioner qualification, or a Masters assessed as equivalent in qualification
A minimum of 5-years experience in Nephrology or renal nursing
Demonstrated ability to practice collaboratively as part of a multidisciplinary health care team, providing high quality patient care
Advanced assessment, problem solving, critical thinking and organisational skills
Excellent communication and interpersonal skills
