Job Description

Join our team as a Care Advisor and make a positive impact on your community by providing compassionate care services to individuals in need.

Responsibilities

Provide comprehensive case management for clients with empathy and respect
Assess client needs and coordinate services within budget constraints
Ensure compliance with organizational policies and legislative requirements
Build and maintain positive relationships with clients, families, and stakeholders
Assist clients in managing care budgets and documentation
Guide and support team members in implementing Anglicare's strategy and values

Qualifications

Certificate IV in Aged Care, Community Services or equivalent
Diploma in Case management (desirable)
Previous experience in Home Care/Community/Aged Care
Intermediate computer skills, including proficiency in MS Office
Current NSW driver’s licence
