Job Description

Exciting opportunity to join a mental health, AOD, and suicide prevention service in Queensland. Lead a team in shaping a groundbreaking community mental health and AOD model.

Responsibilities

Provide leadership and oversight to a team of Stepped Care Coordinators
Support care planning, transitions, and team reviews
Maintain relationships with hub partners and service pathways
Collaborate with the lived experience team for community responses
Provide training, supervision, and support in screening and assessing clients
Guide on mental health referral pathways
Offer high-quality engagement in AOD, mental health, and suicide prevention services
Ensure continuity of care with Better Connect providers
Perform audits and submit reports for quality assurance and training needs

Qualifications

Tertiary qualifications in Social Work, Health, and/or Welfare related sector
Current state-based driver's license
