Job Description

The Care Manager ensures the safety, well being, health, and quality of life of older persons by providing individualized care plans and support services.

Responsibilities

Identifying client needs, goals, and preferences
Reviewing client support plans
Communicating with aged care workers
Maintaining accurate case notes
Documenting assessments and care planning
Ensuring compliance with program guidelines and policies
Engaging in care discussions with health professionals
Supporting clients in decision-making

Qualifications

Bachelor's degree in Social Work, Allied Health, or Nursing
Current APHRA registration preferred
