Job Description

Responsibilities

Provide support to young people in accordance with Youth Services Model of care
Establish effective relationships with clients to encourage self-determination and participation
Provide case management support to clients using TSA case management models
Maintain accurate client records and data
Assist clients in accessing support services and community supports

Qualifications

Minimum Certificate 3 or 4 in Human Services or related field
Experience working with youth desirable
National Police Record check
Current and Valid WA Working with Children check
Current WA Drivers Licence
First aid certificate
